Acute Concussions: What the Body Operating System Reveals About the First 72 Hours

A concussion is often described as a “mild traumatic brain injury,” but for many patients, the symptoms feel anything but mild. Head pressure, dizziness, nausea, light sensitivity, brain fog, neck pain, emotional changes, fatigue, and visual disturbances can appear within minutes or hours after impact. While conventional care appropriately emphasizes rest, monitoring, and gradual return to activity, the Body Operating System (BOS) offers immediate solutions for healing. 

What systems in the body are injured by an acute trauma to the head? 

At Klein Integrative Physical Therapy, we view an acute concussion as a localized structural issue that can cause whole-body problems. The area of impact needs to be reset back to a pre-trauma position, as do all of the structures associated with the impact area. Additionally, the diaphragms, autonomic nervous system and endocrine need to be reset back to neutral. During any physical, physiological or energetically traumatic event, all of the body regulators react to the new trauma. 

Why Symptoms Extend Beyond the Brain 

Many concussion symptoms are driven not only by the impact itself, but by the body’s protective response afterward. Following trauma, the nervous system often shifts into a heightened sympathetic “survival” state. This can create widespread dysfunction throughout the body, including: 

  • Altered breathing mechanics 
  • Increased muscle guarding and fascial tension 
  • Restricted cranial and cervical mobility 
  • Dysregulation of heart rate and blood pressure 
  • Visual and vestibular disturbances 
  • Sleep disruption and fatigue 
  • Digestive changes and nausea 

From a BOS perspective, the body is attempting to protect itself — but prolonged protection can interfere with healing, unless the protective cascade is reset back to neutral.

Why Rest Alone Is Never Enough 

Rest is important during the acute phase of concussion recovery, particularly in the first 24–48 hours. However, prolonged inactivity without restoring system communication can sometimes allow dysfunctional patterns to persist. 

A BOS-informed treatment approach focuses on helping the body regain adaptability and regulation by assessing and treating: 

  • Cranial and dural mobility 
  • Cervical and thoracic mechanics 
  • Visual and vestibular integration 
  • Autonomic nervous system balance 
  • Respiratory coordination 
  • Vascular and lymphatic flow 
  • Global protective compensation patterns 

A Whole-Body Perspective on Recovery 

At Klein Integrative Physical Therapy, we recognize that a concussion is more than just a brain injury. The forces involved in a concussion can create structural strain throughout the body, affecting the cervical spine, cranial system, dura, diaphragm, autonomic nervous system, and connective tissues. These structural and physiological disruptions may contribute to symptoms such as headaches, dizziness, brain fog, visual disturbances, fatigue, nausea, and difficulty concentrating. 

While symptom management is important, our primary goal is to identify and treat the underlying structural dysfunctions created by the injury itself. From a BOS perspective, a concussion will leave behind strain patterns, mobility restrictions, protective compensations, and communication breakdowns between the body’s systems that continue to drive symptoms long after the initial impact. By evaluating and treating these structural impairments—including restrictions within the cervical spine, cranial system, diaphragm, fascial networks, and nervous system—we aim to address the actual sites of injury and dysfunction created by the concussion. From a BOS perspective, the earlier these structural and physiological disruptions are identified and treated, the greater the opportunity to restore normal communication throughout the body and support the healing process. Rather than simply helping patients cope with symptoms, our objective is to correct the dysfunctions contributing to them, reduce the compensatory patterns that develop after injury, and promote recovery at both the local injury site and throughout the body’s interconnected systems. By addressing these impairments as early as possible, we strive to help patients recover more completely and return to the activities that matter most to them.
