Treatment After a Spinal Fusion: Looking Beyond the Surgical Site

Spinal fusion surgery is designed to stabilize a specific region of the spine by permanently joining two or more vertebrae together. While the surgical site may become more stable, the rest of the body must adapt to this change. 

After a spinal fusion, movement does not simply stop. Instead, the body finds new ways to move around the fused area. The joints above and below the fusion may take on additional motion. The pelvis, hips, rib cage, and surrounding soft tissues may also compensate for changes in spinal mobility. Over time, these adaptations can contribute to stiffness, tension, altered movement patterns, and discomfort in areas that may seem unrelated to the original surgery. 

This is why treatment after a spinal fusion should consider the whole body rather than focusing only on the surgical site. 

How Does a Spinal Fusion Affect the Body? 

The spine works as part of an interconnected system. Changes in one area can influence movement and function elsewhere in the body. 

When a segment of the spine is fused, the body must redistribute movement and mechanical forces. Depending on the location of the fusion, a person may develop compensations through the cervical spine, thoracic spine, lumbar spine, pelvis, hips, or lower extremities. 

Scar tissue and restrictions in the surrounding fascia may also affect how forces are transferred throughout the body. Muscles may become overactive as they attempt to provide additional stability, while other muscles may become less efficient because of altered movement patterns. 

These changes can persist long after the surgical incision has healed. 

What Can Treatment Address After a Spinal Fusion? 

Once the surgeon has cleared a patient for treatment, physical therapy can help address the compensations that have developed around the fused region. 

Treatment may focus on: 

  • Improving mobility in areas above and below the fusion that are safe and appropriate to treat 
  • Addressing restrictions in the pelvis, hips, rib cage, and surrounding soft tissues
  • Improving scar mobility after the incision has fully healed
  • Reducing unnecessary muscle guarding and tension 
  • Restoring efficient breathing mechanics and rib cage mobility 
  • Improving strength, balance, and movement control 
  • Identifying compensatory movement patterns that may contribute to pain or limitations
  • Helping the body distribute forces more efficiently during everyday activities 

The goal is not to create movement at the fused segments. Instead, treatment focuses on improving the mobility, stability, and coordination of the rest of the body. 

Why Is a Whole-Body Approach Important? 

Pain after a spinal fusion does not always come directly from the fused area. 

For example, restrictions in the hips may cause the lumbar spine to compensate during walking, bending, or lifting. Limited rib cage mobility may affect movement through the thoracic spine and contribute to increased tension in the neck and shoulders. Changes in pelvic mechanics may influence how forces travel through the spine and lower extremities. 

By evaluating the relationships between different regions of the body, treatment can address contributing factors that may otherwise be overlooked.

Can Manual Therapy Be Used After a Spinal Fusion? 

Manual therapy may be appropriate after a spinal fusion once the patient has healed sufficiently and has received clearance from their surgeon. 

Treatment is modified to respect the fused segments and surgical precautions. The fused area is not forced to move. Instead, manual therapy may be used to address restrictions in surrounding tissues and other regions of the body that are contributing to compensatory movement patterns. 

Because every spinal fusion is different, treatment should be individualized based on the location and extent of the fusion, the reason for surgery, the patient’s healing process, and any remaining precautions. 

Moving Forward After a Spinal Fusion

A spinal fusion changes the way the body moves, but it does not mean that pain, stiffness, or movement limitations must simply be accepted. 

By identifying and treating restrictions throughout the body, improving movement in available regions, and helping the body develop more efficient strategies, physical therapy can support long-term mobility and function after spinal fusion surgery. 

The BOS approach looks beyond the surgical site to understand how the entire body has adapted. Treatment is designed to work with those changes while respecting the stability provided by the fusion and the unique needs of each patient.
